Fourth-Time Revision Rhinoplasty Using Autologous Rib Cartilage and Allogeneic Dermal Graft

This was my fourth rhinoplasty.
After my previous surgery,
I had resigned myself to living with the results
and spent the past ten years without pursuing further correction.
One day, I came across a review on Babitalk
featuring a case by Dr. Hyunjoo Kim at RU Plastic Surgery.
Out of curiosity—but with no particular expectations—
I decided to have a consultation.
After meeting Dr. Kim,
I didn’t feel the need to consult with any other clinic
and immediately left a deposit.
His consultation was incredibly thorough and realistic,
and I felt confident that he fully understood my condition.
The most distressing issue for me
was the elevated columella,
which created an upturned, “pig-nose” appearance.
When I consulted with well-known clinics ten years ago,
they typically diagnosed it as capsular contracture
and emphasized how difficult revision surgery would be,
often in a discouraging manner.
In contrast, Dr. Kim carefully examined my nose
and was the first to point out
that it was not simply contracture,
but rather a short nose deformity—
a much more accurate diagnosis
that immediately built trust.
By reframing the problem,
the corrective approach also became far more precise.
I appreciated that he did not recommend any unnecessary procedures
and clearly explained the realistic limitations
and possibilities of what could be achieved in my case.
For this revision,
autologous rib cartilage
and an allogeneic dermal graft were used.
Postoperatively,
the columella no longer appears excessively retracted,
and the overall nasal line looks significantly more natural.
Although I am still in the recovery phase
and require ongoing care for swelling and redness,
my overall facial balance now appears much softer
and more proportionate,
which has been very satisfying.
Most importantly,
Dr. Kim’s honest and detailed consultation
helped ease the anxiety I had long carried
regarding revision rhinoplasty.
